spawning child sessions sometimes fails with a "custom agent not found" error

What happened?
prompting to create child sessions has been weird for the past few days. I don't know if it's related to a recent update, or with HydraFusion which I've begun using. child sessions randomly fail creation and then the parent session just keeps creating more duplicate child sessions. Sometimes it throws a visible error like:
Created session ‘New session’ (id: 01c0c385-1164-457e-88aa-33f69a5d7dda), but failed to start its Copilot session: RPC error -32603: Request session.create failed with message: session construction failed: Custom agent ‘general-purpose’ not found.
but in other cases it fails silently and creates another session, so I end up with duplicate sessions.
